In 2025, more than 41% of non-US IMGs who submitted rank lists went unmatched. When you also consider Indian doctors who didn’t receive any interview calls, the actual failure rate is closer to 49%.
This is not to discourage you. It’s meant to help you approach the US residency match process with the seriousness it requires.
This guide walks you through everything needed for the 2027 US residency match-from your first USMLE attempt to Match Day on March 19, 2027, including all India-specific requirements.

ERAS and NRMP Are Separate Systems
Many Indian doctors assume there is only one application platform. In reality, there are two-and missing either one means you are out of the match.
ERAS (AAMC): Used to send your application to residency programs. Indian IMGs access it through a token issued by ECFMG.
NRMP: Conducts the match process and collects your Rank Order List (ROL) via the R3 system.
Registering for ERAS does not automatically register you for NRMP. Both are compulsory and have separate deadlines. Your medical college has no role-everything is handled by ECFMG.
Requirements Before You Can Apply
USMLE Step 1 & Step 2 CK
Both exams are mandatory for ECFMG certification. Step 3 is optional but can strengthen your profile.
Important Rule: The 7-year USMLE rule-You must complete all exams and ECFMG requirements within 7 years of your first attempt. Otherwise, your scores expire.
OET Medicine
All Indian doctors must clear OET Medicine.
Even if your education was in English or you scored well in USMLE, this requirement cannot be skipped.
US Clinical Experience (USCE)
Most programs expect at least 2-3 months of USCE.
- Externships are preferred over observerships
- Minimum 3 US-based Letters of Recommendation required
- Indian LoRs are generally not accepted

For first-time applicants, Internal Medicine or Family Medicine are safer options.
J-1 vs H-1B Visa
Choosing the right visa is crucial before applying.
J-1 Visa:
- Sponsored by ECFMG
- Requires a 2-year return to India (unless waived)
H-1B Visa:
- Sponsored by the residency program
- No return requirement
- Better for long-term US settlement
However, Indian applicants should be aware of extremely long Green Card wait times (50+ years under current conditions).
