Breast augmentation, technically known as augmentation mam mo PLA s st y, is a surgical procedure that increases the size and shape of a woman's breast using a breast implant (silicone or saline). The procedure also improves breast contour and volume after pregnancy. A breast implant consists of a silicone gel or saline solution in a silicone casing. A breast implant is different from silicone oil. Most cosmetic plastic surgeons around the world do not inject silicone oil into the breasts to increase breast size and volume. This technique is condemned locally and internationally by most surgeons because the outcome is unpredictable and is associated with numerous complications such as extrusion, skin necrosis and infection. Patients should not allow themselves to undergo this procedure because the risk of unwanted complications is very high, and the result is disfiguring or unpredictable.
Most surgeons use a breast implant that consists of a material that is non-toxic, non-allergenic, non-teratogenic and biocompatible with human tissues, thus having less risk of any adverse results and with a more predictable outcome. It is one of the most common cosmetic procedures performed worldwide, especially in the United States. It is a straightforward procedure with a very immediate and pleasant result. For this reason, the attention and interest of the public has focused on the search for a very safe material for breast implants. Breast augmentation is the only procedure in the field of cosmetic plastic surgery that is surrounded by controversy and scrutiny. In the past, especially in the late 1970s and early 1980s, many health problems such as breast cancer and autoimmune diseases were wrongly associated with the use of silicone implants. For this reason, the US Food and Drug Administration regulated the use of silicone breast implants for cosmetic breast procedures until as recently as mid-2007. After an extensive clinical research study involving more than a thousand women with breast augmentation, it was found that silicone breast implants do not cause any malignancy or breast cancer or any autoimmune disorder.
For this reason, the US FDA has approved the use of silicone implants in cosmetic breast procedures. There are 2 basic types of breast implants, physiological or silicone implants. The implant is composed of an outer membrane formed by a silicone cover. This shell contains either saline (fluid) or silicone (cohesive gel). It is wise for each patient to thoroughly discuss the pros and cons of each type of implant with their surgeon before making a final decision. I perform breast augmentation under deep sedation with local anesthesia or general anesthesia. The procedure can be performed either as an outpatient surgery, or the patient can be admitted or confined for a day or two in the hospital, depending on surgeon and patient preference. Whether the procedure will be performed on an outpatient or inpatient basis, pre-operative laboratory and medical clearance is required. The operation usually takes one to two hours. A breast implant can be inserted through an incision in the armpit or in the folds of the breast or at the border of the areola.
Each choice of incision site has advantages and disadvantages in terms of scar aesthetics and nipple feel. And among the three incisions, the trans axillary approach has the best scar cosmetology because it is hidden, and nipple sensation is well-preserved. However, it is advisable for the patient to discuss this issue further if the haunting ever becomes a significant problem. Swelling and slight bruising will appear after the procedure, which last for several days to weeks and are only temporary changes. The patient must wear a chest belt for 3 to 6 weeks. This will hold the breast implant in place during the healing process and will also help control swelling. Stitches are removed after 7 to 10 days. Any patient scheduled for this procedure will require 10 to 14 days for initial consultation, lab, medical release, surgery; monitoring, suture removal and early recovery. This patient is safe to travel on as the wounds are dry and healed.
