Physical Therapy (PT): Maximizing Mobility

Physical therapy emphasizes gross motor skills, the big movements that use the legs, arms, and torso. The pediatric physical therapist uses specific stretches, resistance exercises, and balance activities to help reduce spasticity, increase the length of shortened muscles, and develop the core strength required for sitting, crawling, and walking.

 

Autonomy is an objective of Occupational Therapy (OT).

Fine motor control and visual-motor integration are concerns of occupational therapy. An occupational therapist can assist your child in learning activities of daily living (ADLs) like putting on a shirt, holding a fork or pencil, and using any type of writing devices. OT empowers them to be physically strong, but also to be independent in the everyday challenges of life.

 

Communication and Speech-Language Pathology (SLP): Communicating Extensively

Many children with CP have delays or problems with speaking and/or chewing and swallowing (dysphagia) because the same muscle groups involved in speaking are also involved in eating. Speech therapy works to build up the oral-motor muscles, to provide communication devices when necessary, and to provide safe swallowing protocols.

Part 3: Modern Management: Medical and Surgical Interventions.

If traditional physical and occupational therapy is insufficient to break the cycle of severe muscle tightness, a pediatric neurologist can use very powerful therapies of modern medicine:

 

Precise Muscle Relaxants: When spasticity is limited to a single muscle group, a doctor can make targeted injections directly into the tight muscle group. These temporary treatments are used to block chemical signals that trigger the spasm, allowing for a 3-4 month period in which the muscle is soft and more susceptible to intensive stretches performed in physical therapy.

 

Intrathecal Baclofen Pumps: If spasticity is widespread and very severe, a tiny pump can be surgically inserted under the skin in the abdomen. This device provides a steady, low-level dose of a muscle-relaxing drug called baclofen into the fluid around the spinal cord to help ease tension and significantly cut down on stiffness in the entire body, with fewer side effects.

 

In certain instances, neurosurgery can be performed on the lower spine, a procedure called selective dorsal rhizotomy (SDR). SDR can permanently stop the "contract" signals from the spastic nerve rootlets in carefully chosen candidates by testing and selectively cutting the specific misfiring nerve rootlets that are causing the spasticity.
