Generally, direct apical fractures will cause more pain than any other type of fracture. The resulting pain comes from parts that put pressure on the bone, making the broken pieces stronger. Almost the only way to get relief from this type of fracture is to get a root canal, as it will soothe the pain and remove the dead pulp of the tooth affected by the fracture.
